Billions Spent, and the Gap Remains. The Cause Is Not the People — It Is the System.
When billions are spent and the gap survives anyway, the usual explanations — budget, effort, competence — do not hold, because all three were present. The CAG has documented the real cause across defence, PSUs, and public projects for years: systems that capture the signal and cannot carry it to a decision. This is the open loop, and it is structural.
